This form is to allow the collection of user information required to allow BFBS to issue TV Systems and renew viewing agreements for our entitled audience. BFBS will use this data to allow us to carry out periodic audits of the conditional access cards and TV receive systems and to confirm user identity when requested to extend the length of the subscription.
