Following the highly critical inquiry report into the death of Baha Mousa, the Chief of the General Staff, General Sir Peter Wall, has apologised to the Iraqi hotel worker's family.

He says the Army is determined a similar episode will never happen again.

He has called it "grave and shameful" and a "shocking episode".
